A referral for inpatient care at MossRehab can be made by you, a family member, your physician, a case manager or social worker. To initiate a referral, contact our Admissions Department.

Once a referral has been made, you will be contacted either by phone or in person by one of the MossRehab clinical liaisons for an evaluation.
Admissions Criteria
All admissions are evaluated individually based on:
- Rehabilitation diagnosis
- Level of medical complexity
- Rehabilitation needs and goals
- Insurance coverage
- Ability to participate in three hours of therapies, at least five days a week
We will work with the referring hospital to facilitate your transition to MossRehab. The MossRehab liaison will advise on which MossRehab unit is most appropriate for you.
