Living Beyond Disabilities Program


At Jefferson Moss-Magee Rehabilitation, we understand that rehabilitation from a disease or injury can be a lifelong journey, marked by ever-evolving levels of ability as milestones are achieved and new goals are set. Serving the needs of individuals at every level of ability has been the inspiration behind the extensive and growing number of programs we offer.

Rehabilitation navigators with the Krancer Living Beyond Disabilities program help individuals at any point in their recovery gain access to the wide spectrum of resources and support available through Jefferson. 

Whether patients are ready to improve upon or learn new skills, increase capabilities or access social and emotional support groups, our specialists can help. We start by evaluating each individual’s specific abilities, listening to their goals, and then connecting them with the right programs, technology, and equipment or professional expertise to get them started.

How the Program Works

Contact

Get started by contacting our rehabilitation navigators at 1-800-CALL MOSS (225-5667) or via email at  livingbeyonddisabilities@einstein.edu. Our experienced rehabilitation navigators will make the first assessment of your goals. You may be directed to a physiatrist—a physician who specializes in physical medicine and rehabilitation—for a more in-depth evaluation.

Connect

Based on your evaluation, we will help you connect with the appropriate professional expertise, which may include physiatrists, neurologists, orthopedists, or physical or occupational therapists; programs; services; or technologies within our extensive network.

Community

To complement your therapy, we have educational resources including rehabilitation-specific newsletters, podcasts and other opportunities for creating community, such as support groups and social programs. Spectrum of Possibilities

Whether someone has survived a stroke or a brain injury, recovered from an amputation, is living with multiple sclerosis, Parkinson’s disease, muscular dystrophy, cerebral palsy or any other condition that requires rehabilitation, his or her needs can range from physical to cognitive to emotional, depending on the specific condition and goals of the individual. We offer extensive programs and services in the areas of functional mobility, language and cognition, mental wellness, social and community connections, general health and wellness and many others.

Functional Mobility

Many of our patients are striving to regain mobility and improve function affected by an injury or disease. At Jefferson, we combine renowned expertise, unparalleled diagnostic labs, pioneering technology and new and innovative approaches in the treatment of every patient.

Technology

We are proud to be the first rehabilitation provider in the U.S. to adopt the tools listed below. The following is an abbreviated sampling of our ever-growing technology offerings:

  • Armeo®Power is a robotic trainer that permits patients with traumatic brain injury, stroke or spinal cord injury to regain use of their arms through precise exercise and 3-D feedback.
  • The G-EO System Evolution is an advanced robotic device that resembles a combination of a stair-stepper and elliptical machine which helps to strengthen a patient’s leg muscles to relearn walking, stair climbing and descension.
  • The RELEAS® hand splint, invented by a Jefferson occupational therapist, helps restore hand function to patients recovering from a stroke or brain injury.
  • ReWalk™ is a motorized exoskeleton suit that enables people with walking impairments to stand, walk and in some cases climb stairs independently.

Language and Cognition

We offer therapy and support for improving communication skills and coping with related challenges during recovery from injury, stroke or disease.

  • The Jefferson Aphasia Center provides a warm and welcoming place where people with aphasia and their families can learn and practice their communication skills in a supportive environment. The Center offers support groups, a computer lab, recreational card games, conversation groups, a Talking Book Club and quarterly education programs.
  • The Stroke Comprehensive Outpatient Rehabilitation (SCOR) program provides outpatient services including physical, speech and occupational therapy for patients who have had a stroke or a non-traumatic head injury. Our treatment teams focus on advancing day-to-day skills needed within the home and community settings.
  • The Neuro-mental Health Clinic provides psychological and psychiatric treatment for people with stroke, traumatic brain injury, multiple sclerosis, anoxic or metabolic brain injuries, brain tumors and any other neurologic injuries. The services are specifically designed to accommodate the added cognitive difficulties that may accompany these diagnoses.

Social and Community Connections

The simple joys of connecting, socializing and integrating are sometimes challenging after injury or onset of a disease. We provide extensive opportunities to help patients connect and participate.

  • The Clubhouse is an innovative program that provides customized skills training and social opportunities for survivors of traumatic brain injury to increase their opportunities for employment and self-sufficient living.
  • We offer support groups for people looking for an environment where they can come together to connect with peers and participate in activities. Support groups for multiple sclerosis, traumatic brain injury, spinal cord injury, stroke survivors, and amputation are just a few of the groups we host.
  • Partnerships with community organizations such as the MS Society, American Stroke Association, Brain Injury Association of Pennsylvania, Amputee Coalition of America, United Spinal Association and YMCA provide extensive opportunities for patients to expand their network of support.

Wellness

Exercise and wellness are paramount to quality of life for all of us, and the specialized needs of individuals with impaired abilities are met in our therapeutic programs.

  • Aquatic therapy helps people with a wide range of situations requiring rehabilitation, such as arthritis or back injuries, as well as people who have had joint replacement surgery. The warmth, buoyancy, and resistive properties of water help reduce pain and stiffness while facilitating movement.
  • The Spinal Cord Injury Maintenance Program is a supervised fitness program that focuses on tailored exercise and healthy living regimens. The program encourages increased fitness and cardiovascular health, and provides an environment for peer socialization.
  • The Multiple Sclerosis and Stroke Maintenance Program are two examples of supervised physical fitness programs we offer for people who no longer require skilled therapy but who would benefit from a continued, supervised fitness program.
  • Various neurological maintenance programs provide supervised physical fitness programs for people who no longer require skilled therapy but benefit from a continued, supervised fitness program.

Jefferson encompasses more than 20 separate sites throughout the Philadelphia metropolitan area, with specialized therapists and resources at each location.

With nationally renowned programs ranging from stroke and brain injury to spinal cord injury, amputation and cerebral palsy treatment and care, our growing network provides a variety of programs and services that support a wide range of patients.
