Refer a Patient to MossRehab

Inpatient Referral

For inpatient referrals, please call our Admissions Department at 215-663-6316.

Referrals can be made by a physician, case manager, hospital discharge planner, or other healthcare professional.

A rehabilitation nurse liaison will conduct an evaluation to determine whether inpatient care is appropriate.  If the patient is eligible for admission, the liaison will help facilitate a transition to MossRehab and will help the patient’s family through the process.

Outpatient Referral

If you would like to receive outpatient physical therapy you will need a prescription from a referring physician. Once you have the prescription you can call any of our outpatient locations to schedule an appointment.
